We're seeking a bilingual (Spanish preferred) Care Manager to join our team on Long Island! What You'll Be Doing:
Empowering Individuals: Provide person-centered care management to individuals with intellectual and developmental disabilities.
Coordinating Care: Oversee and coordinate access to medical, behavioral health, and social services.
Developing Life Plans: Create, implement, and monitor individualized Life Plans with the individual and their support network.
Advocating for Needs: Identify service gaps and advocate for individuals to access necessary services and achieve their goals.
Building Strong Relationships: Work collaboratively with individuals, families, and providers to ensure seamless care coordination.
Ensuring Eligibility: Maintain individuals' eligibility for care management and benefits, including Medicaid and Social Security.
Utilizing Technology: Use electronic health records and telehealth platforms to deliver care effectively.
Providing Transitional Care: Coordinate smooth transitions between care settings, such as from inpatient to home.
Documentation and Compliance: Maintain accurate records and ensure compliance with regulatory standards.
Community Engagement: Travel throughout Long Island to meet with individuals and attend appointments.
What We're Looking For:
A Bachelor's degree with two years of relevant experience, or a Registered Nurse license with two years of relevant experience, or a Master's degree with one year of relevant experience.
Bilingual in Spanish (preferred) or another language in addition to English.
A strong commitment to serving individuals with intellectual and developmental disabilities.
Excellent interpersonal, communication, and advocacy skills.
Ability to work independently and as part of a team.
Professionalism and respect for diverse populations.
