Are you passionate about helping others navigate their healthcare journey? We are seeking a compassionate and detail-oriented Community Health Worker to support individuals in accessing the care they need. In this role, you'll work closely with members, families, and healthcare teams to improve health outcomes through personalized support and care coordination.

Key Responsibilities:
Conduct comprehensive assessments to evaluate members' healthcare needs, functional abilities, and behavioral health status.
Engage with members via phone or video to understand their healthcare goals, identify barriers to care, and provide guidance.
Develop personalized care plans and connect members with appropriate healthcare services and community resources.
Review medical records, utilization history, and member documentation to ensure accurate care coordination.
Identify urgent situations and escalate cases as needed to ensure timely intervention.
Collaborate with members, families, and healthcare providers to facilitate access to healthcare services.
Utilize clinical platforms and communication tools, including OICS, Microsoft Teams, and Outlook, for documentation and coordination.
Qualifications & Requirements:
High school diploma or GED required; a Bachelor's degree in Social Work, Licensed Practical Nurse (LPN) certification, or Certified Nursing Assistant/Home Health Aide (CNA/HHA) credentials are preferred.
3+ years of experience in a healthcare or community health role.
Bilingual in Spanish (Required).
Strong communication, problem-solving, and organizational skills.
Ability to work independently while managing multiple cases effectively.
Proficiency in Microsoft Office, Teams, and clinical software.
Availability to work 8 AM - 5 PM MST.
