Position Title: Evolve LIGHT-FEP Team Lead/SSC1, Licensed Position Number: Employee: Work Unit: Aspire Youth and Young Adults County: Dougherty Immediate Supervisor:Youth Services Coordinator Qualification: Master's Degree in Counseling or other related field Full licensure (LCSW, LPC, LMFT, ECT) or associate licensure actively pursuing full licensure (LAPC, LAMFT, LMSW, ECT) Assigned duties include: oversees staff of this program, maintains caseload of up to 40 youth ages 16-30 who have experienced their First Episode of Psychosis (FEP) within 24 months, provides initial and ongoing assessments, individual, group, and family therapy, collaborates with other designated staff, leads team meetings, utilizes CT-R as the main evidence based practice intervention, meet minimal contacts per month with each individual, provides collaborative documentation, prior insurance authorization completion, attends collaborative meetings with staff and other community stakeholders for continuity of care, assists with crisis response Equipment that includes safe and effective use: None Bloodborne Pathogen Category: Employee performs assigned tasks which involve no exposure to blood or other potentially infectious material AND performance of category I tasks are not a condition of employment (Category III). Include % of time) Provides oversight of program and staff Maintains a caseload of up to 40 youth ages 16-30 who have experienced their First Episode of Psychosis within the last 24 months Covers a region of up to 17 surrounding counties along with another case manager Provides initial and ongoing behavioral health assessments, individual, group, and family sessions mostly in a community setting (some in-clinic) Establishes rapport and regular, consistent interactions with families members Completes all required documentation within 24 hrs. from date of service Meets minimal contacts per month with each individual enrolled Completes all required prior authorizations for insurance reimbursement Maintains a 30% billable productivity Collaborates with CPS, CPS-P, Education and Employment Specialist, MD, nurse, and two case managers daily and consistently Acts as a liaison between agency and contracted agencies Completes all required spreadsheets and all reporting submissions to DBHDD including: Monthly and quarterly monitoring reports Annual and final project evaluation reports Assists with linking individuals to outside supports and resources: Employment School enrollment Support groups Social outlets Physical health activities Assists with attending community stakeholders as well as inner-agency meetings Acts as a liaison between youth, MH providers, and other youth serving stakeholders Establishing/strengthening partnerships with businesses, schools, hospitals Establishing/strengthening partnerships with DFCS, DJJ, Necco, and other agencies Attends all required agency and DBHDD trainings (particularly CT-R trainings) Assists with crisis response including on-call rotation Participates in weekly staffings, doctor days, and assists with weekly groups Treats all with respect and dignity without preferential treatment Communicates accurate information in a professional and courteous manner that conveys a willingness to assist Uses appropriate, established chain of command Follows the office's established practices to adhere to special communication/correspondence, emails, etc. that are of a personnel (human resources) nature Position Title: Evolve Team Lead Position Number: SSC1, Licensed Work Unit: Aspire Youth and Young Adults County: Dougherty Immediate Supervisor:Youth Services Coordinator Qualification: Master's Degree in Counseling or other related field Full licensure (LCSW, LPC, LMFT, ECT) or associate licensure actively pursuing full licensure (LAPC, LAMFT, LMSW, ECT) Special needs of population serviced: Work requires the knowledge and skills necessary to provide direct consumer services appropriate to youth ages 16-30 with First Episode Psychosis/and may have dual diagnoses.
