You will manage a caseload of 30-35 visits, contributing to a highly respected and sizable therapy group in Southeast Texas.

Responsibilities:

• Deliver exceptional physical therapy services to patients in their homes.
• Develop, implement, and evaluate individualized treatment plans.
• Collaborate effectively with a diverse team of healthcare professionals.
• Ensure compliance with home health regulations and standards.
• Document patient progress thoroughly and accurately.
• Educate patients and their families on physical therapy methods and techniques.

Requirements:

• Must possess a current Physical Therapy License.
• Ability to work independently and as part of a team.

• Willingness to work flexible hours, including weekends as needed.
